If you are an adult in Washington State with limited income and resources, you may qualify for Long‑Term Services and Supports.

Washington Apple Health is Medicaid. It helps pay for long‑term services and supports for people with very limited income and resources.
The person who needs care must apply for Apple Health before a caregiver can be paid. Apple Health can only cover personal care if the person applies and meets eligibility rules.
If you’re eligible, Apple Health Long-Term Services and Supports can help pay for care in:
- Your own home
- An adult family home
- An assisted living facility
- A nursing facility
How to Apply
Choose one of these ways to apply for Apple Health Long-Term Services and Supports:
- Apply online on the Washington Connection portal website; OR
- Call the Home & Community Services Single Application Phone Line at 877-429-7019; OR
- Download and submit an application; OR
- Contact your local DSHS Home & Community Services office.
What Happens After your Application is Received
- A Public Benefits Specialist speaks with the person or their representative to review income and resources. This phone interview helps decide financial eligibility.
- A social worker then schedules an in‑home assessment to see if the person qualifies based on their care needs.

Need Help?
Staff at your local office can help you fill out the application. They can also provide materials in Braille, large print, translated languages, or assistive listening formats. Interpreters are available.
If you want someone else to apply for you, you’ll need to sign a consent form giving them permission. This form must be updated every year.
