Applying for Long-Term Care Coverage
If you’re an adult in Washington State with limited income and resources, you may qualify for a health insurance program that helps pay for long-term care. It’s called Apple Health Long-Term Services and Supports, and it’s designed to help people who need daily support due to age, disability, or health conditions.
What It Covers
If you’re eligible, Apple Health Long-Term Services and Supports can help pay for care in:
- Your own home
- An adult family home
- An assisted living facility
- A nursing facility
How to Apply
You can apply if you are age 18 or older. There are two main ways to apply:
Option 1: Apply through your local office
- Call or visit your local Home and Community Services office.
- Ask for the application form to be mailed to you, picked up in person, or downloaded online.

Option 2: Apply online
- Go to Washington Connection and click “Apply Now”
- If you are under age 65 and not on Medicare, visit Washington Healthplanfinder and request long-term services and supports.
Need Help?
Staff at your local office can help you fill out the application. They can also provide materials in Braille, large print, translated languages, or assistive listening formats. Interpreters are available.
If you want someone else to apply for you, you’ll need to sign a consent form giving them permission. This form must be updated every year.
